Instructions for State Agencies (only) served by the Office of the State Controller to Receive W2-C Statements to Correct W2's Issued to Employees with an Incorrect Social Security Number or Incorrect Spelling of Names
If an employee of an
agency served by the Office of the State Controller receives two or more
W2 statements with different social security numbers or incorrectly spelled
names, the employing agency can request a W2-C statement be issued
for them. The payroll office will need to do the following:
- Get a clear xerox copy of the employee's social security card.
- Make a clear copy of all W2 statements issued to the employee.
- Send the request and supporting documentation to Carol J Stephenson at the Office of the State Controller by mail or by fax.

Carol will produce and issue a W2-C in a timely manner. The form will be mailed to your agency.
Note: The copies which
are sent to Carol MUST be completely
clear.DO NOT send the original forms
to our office. The employee must file their taxes with the original W2
forms as well as the W2-C form.
