In order to ensure a supply
of W-2 envelopes, this years' envelopes were again ordered by our office.
Please
order any needed envelopes
through our office as follows:
- Enter your agency number, the name of the person ordering the envelopes and the quantity of envelopes needed into the spaces provided below. The quantity ordered should be rounded to the nearest 100 envelopes.
- Make a copy of this memorandum, which shows the quantity of envelopes needed by your agency, and return the copy to this office to the Attention of Roger Farmer. As an alternative, this form may be faxed to (919)981-5570.
- Upon receiving your order, the envelopes will be provided and an invoice will be prepared and mailed by the OSC Fiscal Section back to your agency. The envelopes will be invoiced at the rate of $7.50/100 envelopes.
