The payroll transmittal includes
three main changes. First, we have deleted the deductions for the
three remaining HMO Plans. There are no HMO Plans authorized for
the new plan year. We have provided a listing of these deletions.
Your office may wish to contact these employees to ensure that they remember
to enroll in the State Health Plan. Second, we have changed the 800
series to their respective 400 series code, for those employees who had
filled out a rejection form. We have provided a listing of these
employees. In order to again have their deduction changed from an
exempt to a non-exempt status, a new rejection form will be required.
Third, we have changed the hospital premiums to reflect the proposed new
rates, as per the attached rate schedule. A listing has also been
provided for these changes.
