Enrollment and Forms - Merchant Cards

On August 1, 2006, the Office of the State Controller executed a new Master Services Agreement (MSA) with SunTrust Merchant Services. Following is information and instructions for eligible entities desiring to enroll as a participant under the contract. Information and forms are also available for current participants desiring to add additional outlets or applications, or to conduct certain maintenance functions.
